• A request by a pharmacist (dispenser) for a formal prescription from a prescriber (e.g. physician). The prescription requests legitimatizes the dispensing of therapeutic goods following an instruction (e.g. through phone conversation or standing written agreement) from a prescriber. It has essentially the same contents as a prescription document except the pharmacist fills it out as a template for the doctor to sign. Information provided in the document includes patient identifiers (patient ID, name, address, etc.), pharmacy information, prescriber instruction details (prescription details, mode of communication), the prescription item and formula/dose information, and any notes from the pharmacist.
