To determine if the patient has cognitive impairment, the patient's informant is asked six questions about how the patient is compared to when s/he was well (e.g. 5 - 10 years ago). The total score is equal to the number of items answered 'no', 'don't know' or 'N/A'. If the patient scores between 0-3 out of 6, cognitive impairment is indicated.
